The State Innovation Model Initiative (SIM) is an initiative of the Center for Medicare and Medicaid Innovation (CMMI).  CT’s model design grant requires the state to produce a state healthcare innovation plan (SHIP) and a model of healthcare delivery and payment reforms that will reach 80% of Connecticut’s residents within 3-5 years.  The leaders of the plan are looking to meet for 2 hours once with 8 to 12 Family Physicians to review the proposal and to get their input and feedback.  This meeting would occur in the evening during the first week of December.  Their preference is to have Family Physicians who either don’t yet have an EHR or do have an EHR but not yet pursued PCMH certification as these are the groups they feel will benefit most initially.
